Evidence before outrage

The accountability test

  1. Identify the resident impact—not just the F-tag.
  2. Separate what surveyors observed from what was alleged.
  3. State scope, severity and survey date in plain English.
  4. Look for the facility's correction and what changed afterward.

The fairness rule

A survey finding is a dated record of conditions observed at that time. It is not proof that the same conditions exist today. Facility responses, plans of correction and later surveys belong in the story whenever they are available.
